Referral
Most patients are referred by their primary physician.  Referrals from other sources will be confirmed with the patient's physician.

Payment
Columbine Home Health, Inc is a Medicare and Medicaid certified agency.  Many private insurers cover Home Health, and we accept private pay.

Qualifications
The patient must be homebound to receive Medicare home health services.  A patient is considered homebound if: leaving the home is a considerable and taxing effort, leaving the home is infrequent, and of short duration.  Call our office for clarification if you are unsure of the homebound status of an individual.  A physician orders home health care and supervises the care plan.  The condition must be expected to change toward improvement or decline and it must warrant the skilled level of a registered nurse, physical therapist or speech language pathologist.  Patients who are not homebound may receive outpatient therapy services in their own home.
