Skilled Nursing Skilled Nurses assist with Medication, Pain Control, IV Therapy, Ostomy Care, Diabetic Teaching, Wound Care, Case Management, and Resource Referral Home Health Aides Home Health Aides are regularly supervised by an RN and assist with bathing, skin and wound care, and exercise programs Physical Therapy Physical Therapists assist patients to function independently or within the limits of a physical condition including: Joint Replacement Rehab, Stroke and Neuro Rehab, Range of Motion, Home Safety Management, Gait/Balance Rehab, Muscle Re-Education, Pain Relief, Fall Risk Occupational Therapy Occupational Therapists assist patients to resume activities of daily living including; Dressing, Preparing Meals, Home Safety Management, Splinting Speech Therapy Speech Therapists assit patients with Communication and Swallowing Social Worker Medical Social Workers assist the patient and family in planning for long term care including; Financial Arrangements and Guidance and Counseling
